Mobile Dentistry Services K-5

Partnering with the Smiles Program, Keystone Elementary and Middle School will offer mobile dentistry services for our students. State licensed dentists will come to our campus during school hours on March 2 and 3.  

Appointments are limited, and students are served on a first-come first-serve basis.

There are two ways to register: complete the form electronically @ https://www.myschooldentist.com/Default.aspx or obtain, sign, and return a paper copy of the form from the school office.

During the appointment, a state licensed dentist will check your child's mouth and teeth, as well as provide a cleaning, x-rays as necessary, fluoride treatment and apply sealants, as needed. Additional care, such as fillings, may also be provided. A dental report card will be sent home with your child. By completing this, the dentist will be able to see your child for their initial visit as well as all follow-up visits.
