WELCOME TO KEYSTONE!
Open enrollment for non-district residents is open from April 15th through May 29th. No late applications accepted after the May 29th deadline. NO EXCEPTIONS. Acceptance is not guaranteed.
Applications for first time applicants must be completed and turned in to the Board of Education in person. Returning open enrollees may return their applications to their building secretaries or in person to the Board of Education.
Acceptance letter for grades 1-12 will be mailed out by mid-July. A confirmation letter will be included and must be returned to the Board of Education by the deadline listed in the letter. Kindergarten acceptances will be made by mid-August. Letters will be sent out via mail and/or email.
Once accepted into the Open Enrollment Program, registration on Final Forms for first time applicants can begin--See yellow box for required documents. Returning open enrolled students can update their Final Forms after July 1st.
All applicants (new and returning) must provide two proofs of residency, a recent official transcript or record of achievement, a copy of student’s IEP/504/ETR (if applicable), and records of any suspensions or expulsions from the current and previous school term.

ENROLLMENT
All registration are done through Final Forms located on the Keystone Local School District website.
New registrations for grades 1-12 for the 2026-2027 school year will open on May 1st, 2026. Please ensure you are using the correct school year when registering!
Once you begin registration in Final Forms, you have ten (10) days to finalize or your application may be deleted.
VISITOR ID REQUIREMENT
All visitors to district buildings must present a valid driver's license. As part of our security procedures, prior to entry, you will be required to scan your ID as part of the sign-in process. Please plan accordingly when visiting any district building to submit your application.
REQUIRED DOCUMENTS
FOR ALL REGISTRATIONS IN FINAL FORMS
Two (2) proofs of residency dated within 30 days.
- Home Owner - Tax/Mortgage statement, Insurance statement/declarations page, pay stub, bank statement, signed purchase agreement with occupancy date, utility bill
- Tenant - Signed lease agreement with property owner
- If parent/guardian resides with student in home of family member or friend , that person must fill out a Residency Verification Form and have it notarized and provide two proofs of residency in their name.
- Child’s birth certificate or passport Student’s Immunization Record Parent/Guardian’s Drivers License or State Issued ID card
SPECIAL CIRCUMSTANCES
Custody Decree- Complete, current legal documentation must be presented at the time of registration. Upload any future revisions to Final Forms.
Guardianship-Complete, current legal court documents must be presented at the time of the of registration. Upload any future revisions to Final Forms.
Special Education paperwork--Current ETR/MFE and IEP for students with disabilities, or the current Section 504 Accommodations plan, or Gifted Identification information must be presented at the time of registration, if applicable. An official request will be made to the previous school for these documents as appropriate; however, having these documents upfront facilitates a smoother transition.
Notarized Residency Verification Form-For situations when the parent may live with someone in the district and not have proof of residency to supply. Link to this form is located in Final Forms on the New Student Registration Process Form.
