KHS New Student Registration Forms
What you will need to register your student (s) at Keystone High School:
- Birth Certificate**
- Immunization Records**
- Custody papers if applicable**
- Proof of Residency (driver’s license, copy of any utility bill, phone/cell bill, purchase agreement, etc.) Document must prove residence within the Keystone School District.**
- Social Security number
- Copy of student’s Transcript
- Copy of most recent report card
- IEP/ETR if applicable (special needs students)
** - means we must have a copy of these items in order for the student to begin classes at KHS.
When you have all of the above information and you have your forms filled out, please contact the High School to set up an appointment with the appropriate Guidance Counselor at 440-355-5132 .
Please remember that all of the 3 forms below must be completed.
